FBCE Liability Waiver 2021/2022 School Year

First Baptist Church of Enterprise designates the abbreviation “FBCE” for its name throughout this document.

I (we) hereby give permission for my (our) child to attend and participate in activities sponsored by FBCE. I (we) hereby authorize FBCE to include my child in supervised water activities. I (we) authorize FBCE and its acting leaders to teach and lead my (our) child in religious lessons and services, which may include prayer and Bible teaching. I (we) hereby release, forever discharge, and agree to hold harmless FBCE and the directors thereof from any and all liability, claims, or damages for personal injury, sickness, or death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned and child-participant that occur while said child is participating in any trip or activity with FBCE. Furthermore, I (we) [and on behalf of my (our) child-participant if under the age of 18 years] hereby assume all risk of personal injury, sickness, death, damage, and expenses resulting from my (our) child’s participation in recreation and work activities involved therein. Further authorization and permission is hereby given to FBCE to furnish any necessary transportation, food, and lodging for my child. The undersigned further hereby agrees to hold harmless and indemnify FBCE, its directors, employees, and agents for any liability sustained by said child-participant including expenses incurred attendant thereto.

I (we) hereby authorize FBCE to transport my (our) child to, from, and during FBCE church activities including transportation to offsite locations or part of camps, retreats, conferences, and mission projects related to any FBCE sponsored event. I (we) authorize FBCE to take or authorize transport for my child(ren) for medical treatment in the event of illness or emergency. Should it be necessary for my (our) child to return home due to medical reasons or otherwise, the undersigned shall assume all transportation costs.

I also give permission for pictures/videos to be taken of my child/ren at and during FBCE events. I also give permission for said pictures to be used in manners to include but not limited to FBCE publications, FBCE advertising, FBCE social media/internet, church services, and in classrooms.

I (we) hereby authorize any adult in whose care my child has been entrusted by FBCE to consent to any medical care or treatment to be rendered to the minor under the supervision and on the advice of any licensed medical professional, whether such diagnosis or treatment is rendered at the office of said medical professional, urgent care facility, or at a hospital. I (we) hereby authorize any licensed physician or medical treatment center to treat my (our) child in the event of an illness or emergency. The undersigned shall be liable and agree(s) to pay all costs and expenses incurred in connection with such medical and/or dental services rendered to my (our) child while in the care of FBCE.

It is the responsibility of the parents/guardian to notify the church of any changes in medical condition, insurance, guardianship, address, or phone changes. The medical consent and liability waiver provisions hereof shall remain in full force through December 31, 2022 or until the child is no longer a child-participant in the organized activities of FBCE.
